Unknown. Possibly of Etruscan origin. The suffix seems similar to that of pedes, eques, veles, comes, but the origin of mīl- is opaque. A connection to mīlia (“thousands”), perhaps as "person going by the thousand(s)", is difficult to confidently motivate semantically.
